The Process
Step 1: Call the Office to Provide Demographic Information
​Submit our patient intake form. Dr. Chung will reach out out to you via email 48-72 hours after submission.​
Step 2: Complete Your Medical History
We will email you a link to complete your medical history.
Step 3: Schedule Bloodwork and Virtual Visit
Once you submit your history, we will contact you with an order for bloodwork & schedule you for a virtual visit (for which you must be in Maryland or Virginia) to review your results and options.
Step 4: Complete Bloodwork
We will submit a lab requisition, and you should complete your blood work no later than 14 days before your scheduled virtual visit.
Step 5: Virtual Visit
a) Review videos prior to telehealth visit
b) Telehealth visit with Dr. Chung to review your personal plan

Step 6: Decision to Proceed with Pellet or Non-Pellet Therapy
